The Pt Comes In And The Doctor Does Excision Of Furuncle Along With Sebaceous Cyst That Was Next To The Furuncle. He Want's To Use The I&d Code And Destruction Of Lesion Code.

Example: 10060 I&d
17000 Destruction Of A Lesion

Dx 1. Fununcle
2. Sebaceous Cyst

In His Dictation He Does Not Mention He Did I&d, Only That He Did Excision Of Furuncle And Sebaceaus Cyst. He Also Used Em Level 3. If This Ok, Would You Use Modifier 51 Or 59 With 17000 Code. And I Know To Use Modifer 25 With Em Level.
