I need proof to an answer, but I don't know where to find it online:
PreOp days/Global to Surgery

Pt was seen by Plastic Surgeon for a consult on 9/4/2009 with dx for surgery

Pt came back to Plastic Surgeon for another visit on 9/18/2009, same dx as previous visit (what code should I bill for this visit, the surgeon gave me a code 99022 which is not a valid code and I don't have the office notes)

(Pt went to PCP on 9/23/2009 for preop clearance)

Pt will be having Surgery with in the next few weeks (not the day after 9/18)

Can I bill an established E/M code, 99212-99215, for 9/18/2009?
Do I have to worry about any preop global days?
Modifier? Or is there another code I should be looking at?

You would use the 99212-99215 code set. I would pull the note first though, because I am wondering if maybe the surgeon meant 99024 and wanted this to be a no charge visit.

I agree that it should be a 9921X code for the 9-18 visit. The fact that the patient went to their PCP doesn't affect your billing at all. You are only responsible for your doctor.
