Wiki Need help with 95951 and 95957

In need of help with CPT 95951 and 95957.
Has guidelines for these two codes changed over the past year? 2014 to present?
My claims are being denied for code 95957 for being inclusive to 95951.
Anyone have any tips on how to have insurance pay both codes as two separate codes.
