Procedures Performed

Iliac Angiogram
Left lower extrem angiogram
Right lower extrem angiogram
Orbital atherectomy of the left proximal and distal superficial femoral artery using a 2mm Classic Crown at low, med and high speed.
Balloon Angioplasty of the proximal superficial femoral artery with a 5.0 x 4.0 mm balloon at 4-5 atmospheres
Intra-arterial nitroglycerin.

Can you post the actual procedure note? Coding just from this may not be completely accurate. But in looking at what you have, some codes to consider would be:

37225
75716, 26,59
75774, 26,59 (maybe if the iliac angio was an additional selective branch)

Cath placements are now bundled as is S&I, so you should only bill the 75716/75774 if you meet criteria on pg. 373 of AMA Professional CPT.
