ANy help with the following administration codes would be greatly appreciated. I am not sure about coding for the J1100/J2469 given at the same time or the J3475/J3480 given at the same time.

Patient given the following:

Dexamethasone Sodium Phosphate (J1100) 09:48-10:22
Palonosetron HCI (J2469) 09:48-10:22
Emend (J1453) 10:22-10:53
Magnesium Sulfate (J3475) 10:53-13:02
Potassium Chloride (J3480) 10:53-13:02
Docetaxel (J9171) 13:02-14:13
Mannitol (J2150) 14:13-15:27
Cisplatin (J9060) 14:20- No stop time documented
Sodium Chloride (J7040) 15:27-15:58

I have sent to the office for an addendum for the stop time of the Cisplatin.

Thanks in advance for any help on this. Infusions still confuse me. :confused:
 
This is how I would code the scenario

J1100 & J2469 96367
J1453 96367
J3475 & J3480 96367 & 96366
J9171 96413
J2150 96368
J9060 96417 + 96415 for an additional time
hydration 96361
 
The nurses need to document that the drugs were admixed so you know specifically that the drugs were not ran concurrent.
