Good morning,
A middle aged female with cough and SOB for 5 weeks, and h/o asthma, visits physician. After detailed ho and exams( exam of RS shows decreased breath sounds, no ronchi/wheeze), physician writes dx as resp infection with risk of pneumonia. Cxr. Should i code as a/c lower RI or as unspecified (J98.8)

Following the ICD-10 guidelines by using the alphabetic index first, 'respiratory infection' ('infection' --> 'respiratory') takes you to J98.8. You can only get to the more specific J22 code if there is documentation by the physician that this is either 'acute' or 'lower'. From what you shared of the record above, I don't see either of these terms, so J98.8 would be correct.
