The patient delivered at 20 weeks, vaginally. This is not a 59812, nor a 59820, nor a 59821 as it was not surgical. Was not induced, is not global (only 4 visits). Is it billed as a 59425 (4 antepartum visits) and the 59409 for vaginal delivery?

Wouldn't you use a 59410, vaginal delivery including postpartum care? The v code would show that the fetus didn't live.

You would bill 4-7 antipartum visits. No delivery code. I believe for missed ab its 632 for your icd9

if the patient delivered the baby then it is not a missed AB. At 20 weeks it is a delivery if the patient did as you state, deliver the baby vaginally. It would be a code for the delivery plus a code for the outcome as stillborn. It is a spontaneous abortion prior to 20 weeks, it is a missed AB if the fetus is still inutero. At 20 weeks and over if the patient delivers the fetus without any assistance then it is a normal delivery.
