Wiki need help with modifier- xu or 59 with diagnostic cath and pci same day

i work for hospital and do facility billing/coding. for medicare patients diagnostic cath (93458) goes out with rev code 480 and for commercial insurance diagnostic cath goes out with rev code 481 and also if same day pci is done i append xu on 93458 . question is - should i append 59 modifier to diag cath 93458 with pci same day will it make any difference in rev code or payment? since xu is one of the categories of 59
