Need help with post op modifier

My doctor did a post op nasopharyngoscopy - I coded it with a 58 but Medicare denied it with reason code of inconsistent modifier or required modifier missing. Would this be coded with a 59 instead? Any help would be appreciated!

Was this done in the OR? If so, look at modifiers 78 and 79.
