I need help with coding the following
FINDINGS: A single gunshot wound to the abdomen. Exit wound, posterior
thigh, left with crossing on the midline. There was an injury in the small
bowel enterotomies x4. There were clusters in one area in the pelvis. These
were resected en bloc with primary anastomosis. The injury to the bladder was
a single gunshot wound to the dome of the bladder that was opened and closed
in layers. There was a blood in the urine at the place of the Foley. The
evaluation of the rectum, no blood at the opening and no blood on rigid
sigmoidoscopy, so procedure would be exploratory laparotomy, resection and
primary repair of small bowel, open repair of the bladder in layers,
proctosigmoidoscopy and placement of Foley.
Now I came up with 49000, I can not decide which code between 44603 and 44202. The next code I think is right is 51865, then 45327. Can anyone tell me if I'm even close?
Does the bowel enterotomies x4 require me to use the add on 44203 3 times?
