Pt was seen in the office as a group meeting...the ARNP, PhD, LMSW, pt's pastor and the Pt were all present. No time was noted in the chart. Pt and the pastor contributed info. The ARNP, PhD and LMSW all discussed options with the pt, resources for treatment, and medications she was currently on. I am completely at a loss of what I should code this as? Would it be a timed code if they do an addendum and add time (90804-90808)? would it be Psychotherapy group (90853)? Or would it be a Family Psychotherapy w/Pt present (90847)? Any ideas or help on this would be great. The payers are Medicare and XIX
 
I would just code individual therapy (have an addendum done with time). You can't bill group because there isnt multiple patients making a group. I wouldn't bill family because the focus of this therapy is NOT the family and the impact the patient's illness has on family. That really only leaves you with individual.
