Can someone explain this answer to me....question is : patient presents to Dr office with cough, headache, and congestion. What ICD-10-CM code do you use?
A. J98.8
B. R05, R09.89, R51
C. J02.9
D. R05, R09.89, R51, J98.8

I answered A they told me wrong and answer is C. Can anyone explain? Thanks

The correct answer is B. If all you are given is symptoms, then that is all you can code. The coder is not allowed to determine the diagnosis given a symptom set.
