You would only use 99211 since the pt.was seen by the nurse only.

You may not use a 99211 for a blood draw even if the nurse takes vital signs. A code exists for blood draws and that is the code you are to use and you may not code both. The patient was schduled to come in for the purpose of the blood draw so there is no rationale for the 99211.
