Wiki need it in writing

I need help, I know the answers but my doc wants in writing. He doesn't want to here it from me. so I hope somebody can tell me where to find it.
1) Is it ok to bill fx care if the pt is only seen once and then pt is prn'd ?
2) If there is a question of a fx, but the doc is providing the same treatment to the pt as he would if it was a positive fx can he bill fx care?
Again he needs to see the answers in writing.
