Two of my physicians in the same practice worked together to perform a TSA. One is listed as the primary surgeon and he dictated the note. The other physician is listed as the "co-surgeon" on the OP note. Does this one note cover both physicians or do each need to dictate their own note?

When each surgeon is performing part(s) of the same surgery simultaneously with the same or different specialties, medical record documentation should show the services of each surgeon and the medical necessity of two surgeons

Medicare will always require documentation when the Medicare Physician Fee Schedule (MPFS) Relative Value File (RVF) (also known as the data base) has an indicator of "1." The second claim will suspend for review and we will request documentation

If the PFS RVF has an co-surgery indicator of "2" and both surgeons have different specialties, Medicare can allow payment without additional documentation
