Hello,

I need to have one help. could you let me know that LCD & NCD criteria would be consider in only Medicare & Medicare HMO. either Medicaid & its HMO plans & including all commercial plans.

LCD's & NCD's are used only by Medicare. Other commercial carriers, whether HMO or PPO, may have their own coverage determinations and you may be able to find those online for each plan. I'm sorry, I do not know much about Medicaid.
