Wiki Needle Localization breast biopsy x4-HELP

I'm coding an OP note where the doctor performed a needle localization breast biopsy on 4 lesions. I know the codes would be 19125, 19126, but I'm not sure how to correctly list the additional lesions. Would it be 19126 x3?
