Impression:
1. No residual stone is identified post percutaneous
nephrolithotripsy.
2. Successful removal of nephroureteral stent.
3. Patient complained of oral thrush and was treated with oral
nystatin he has been instructed to follow up with Dr. Nelson

History: Status post percutaneous nephrolithotripsy

Anesthesia:
1. 1% lidocaine.

Contrast: Iso-250: 30 ml. Fluoro Time: 2.4 min.

Procedure: Informed consent was obtained. Multiple overhead
radiographs were obtained in conjunction with injection of contrast
into a left nephrostomy. Upon completion of the nephrostogram the
nephro ureteral stent was removed. Completion radiographs were
obtained.

Findings:

Initial radiograph:

Stones: none
Antegrade pyelogram:
Stone burden: none
Calices: intact
Pelvis: intact
UPJ: intact
Ureter: intact
 
My coding software says the 50394 is bundled; and since it's not a separate session or area, I wouldn't think you could use the 59. I would keep the radiology code, though.
