If a patient is seen at an outpatient Nephrology Clinic, would the primary diagnosis be CKD or would it be hypertension with CKD? I am seeing charts with both coded as diagnosis.

It would be hypertensive CKD (I12.-), followed by the CKD stage.

I thought so. Then I thought maybe it was different because the patient was being seen at a Nephrology clinic.
