We have an NP the is billing Medicare 3 units of 64450 and is frequently getting denied. This is being done for pain management and ther are 3 separate branches being injected. Our contractor is Noridian. What is the best way to bill this?
 
Medicare only pays for 2 of these. If the block is in another location, you might be able to use another code for the 3rd test.
