I'm having a hard time learning which nerves may go with 64400 (trigeminal nerve or division or branch) 64402 (facial nerve) and 64450 (other peripheral nerve or branch). My providers consistently document injecting into the "ZMT nerve" - have searched high and low and am not sure what this one is. Some material I've read states that the auriculotemporal nerve falls under trigeminal, and another source said it is facial.

Another question I would like clarified, as I have seen a couple of threads stating both:
When a 2 nerves are injected that are trigeminal is it coded as:
64400 x 2 OR 64400, 64400-59.
What about if there are 3 to 4 nerves that are all trigeminal?

We report 64400 and 64400-59 per additional divisions or branches of the trigeminal nerve. Of course it would depend on the payer also and how they want it reported.
