My physician performed a Median nerve decompression on the right carpal tunnel and then created another incision at the elbow and released the median nerve there too. My question is do I code 64721 with 22 mod or 64721 and 64708 because I'm not finding a code for decompression of a Median nerve at the elbow? From what I can tell 64721 involves the wrist so any other insight would be great.
He also performed a right partial trapezium excision and extensor carpi radialis longus tendon transfer on the right CMC joint post arthroplasty, thoughts?

Query your physician. Did he really do a Median Nerve Decompression/Neurolysis at the elbow, or did he do an Ulnar Nerve Decompression/Neurolysis or Transposition at the elbow? Median Nerve Decompression at the elbow would be very rare.
