Dr performs repair/suture of both the motor branch of the brachialis AND ulnar nerve fascicle with operation microscope. I'm thinking:

64861 - brachialis nerve
64872 - ulnar nerve
69990 - microscope

Any suggestions? Thanks.
 
Well you said repair/suture of both brachial and ulnar motor nerves so that would be two separate codes. There are no edits also.
Also, I don't think that add on code 64782 would work,(without reading the op note), because that is for suture of the proximal and distal nerves where repair was delayed because the initial wound was contaminated. It just doesn't seem like that would work for an ulnar motor nerve.
