Having issue getting paid for Nerve Study codes. We are being advised by Medicare a required modifier is missing? Any suggestions or where to point me in the right direction?

Diagnosis billed: R20.2 M79.621 M79.622
CPT 95912 x 1 unit
CPT 95886 x 2 units
