If a carpal tunnel revision is performed with a nerve wrap, what code(s) would you suggest if performed at an ambulatory surgical center?

Nerve wrap with revision of CTR

I use unlisted code 64999 compared to 15777 since nerve wrapping isn't included in 64721 according to this source from Find-a-Code https://www.findacode.com/newslette...re is,Current + Archives/Advanced Coding Pack. There is also a CPT note under 64721 that says "For neuroplasty with nerve wrapping, see 64702-64727, 64999"

The nerve wrap is included in 64721, however a revision could be billed with a 22 modifier if supported in the documentation. Revision typically involves excision of extra scar tissue from the previous procedure resulting in extra time and work. Providers need to be very clear in the documentation to support the 22. Look to see if the payer has a specific policy on requirement for 22 modifier.
