Wiki Neupogen Dx

Could anybody help me with the correct Dx to code for Neupogen per Medicare LCD? The Medicare reps have given us two different answers. One says we need a primary AND secondary, while the others say we need just one Dx. Please advise.

Your primary code should be either V58.11 or V66.2 depending on when they received their chemo treatment and your secondary code should be the cancer dx if they are receiving it to reduce incidence for neutropenia.
If they are receiving it for nuetropenia w/ MDS & other specific diseases use the following:
Primary Secondary
288.02 238.72
288.03 + 238.76
288.09 238.79
340

The following dx codes can be billed alone:
288.02, 288.03, 288.09

If the neutropenia is an adverse effect of the chemo then you code the neutropenia first with the code for adverse effect of chemo ( E code) secondary
