What would be the appropriate diagnosis for Falling? Pt has Parkinson Disease and falls alot.

Coding Clinic 4th Q 05 "New code V15.88, History of fall, has been created for patients who have fallen in the past, and may be more susceptible to falling in the future. It therefore also includes the concept of a person being at risk for falling. The intent of this code is to capture data on people who experienced a fall due to tripping or slipping (with or without a known injury) and who, because of age or infirmity, may fall again. These people are considered to be at risk for falling. Since falls can cause serious injuries, fall prevention is important. This code was created to assist geriatricians and occupational and physical therapists in classifying the problem for which they are evaluating a patient. Although the intent of creating this code did not include its use for patients who chronically fall due to conditions such as Parkinsons disease, or other conditions that cause instability, toddlers who naturally fall a great deal, or healthy individuals who have fallen while playing sports or other activities, this code may be used for such cases when the history of fall is recorded in the medical record, and thus a clinical concern about risk for falling is present.


Code V15.88 is being added to the V Code Table under the first or additional list because assessment of the risk for fall may be the reason for the encounter. This is an exception to most of the codes within category V15. Though it is a history code its function is that of a status code."
