How to code this? I have been using unlisted procedure. My reasoning for using unlisted is that I've always believed that craniectomy/craniotomy section of CPT represented "open" procedures, and the neuroendoscopy section of CPT does not have a code for craniosynostosis. I have a doctor who insists that the craniosynostosis codes in the craniectomy/craniotomy section can be used for open or endoscopically assisted. I can't find any definitive literature to support either postion, but the fact that there is a neuroendoscopic section of CPT that has procedures that are the neuroendoscopic equivalent of the "open" procedures in the craniectomy/craniotomy section kind of supports my position. No? Other opinions? Links to definitive literature?
