Provider want to bill code 22551 with 22859 (new for 2017). Medicare is denying indicating code 22551, is not accepted per CMS guidelines. I'm not able to locate documentation that states CMS does not always follow AMA coding guideline.-Physician want to see something that supports that CMS will not pay for 22551, as a primary code, even though CPT book allows for billing of that code as
