Is bupivacaine a neurolytic substance? I am trying to determine if correct billing for this procedure is 62282/ 77003 or 62311/ 77003.

Indication: Back pain, leg pain, lumbar radiculopathy, 724.2 and 724.4. Procedure: The patient was informed and consented for the procedure then placed in the prone position on the fluoroscopic table. The patient was prepped and draped in a sterile

TECHNIQUE. 1% lidocaine without epinephrine was utilized as a local anesthetic. Under fluoroscopic guidance, a 22-gauge Touhy needle was advanced from a posterior approach and into the L4-5 posterior epidural space. Needle tip position within the posterior epidural space was confirmed with injection of 2 mL of Isovue-200 M. A lateral fluoroscopic image was sent to PACS for documentation. This was followed by injection of 2 mL 1% lidocaine without epinephrine, 2 mL of 0.5% bupivacaine and 80 milligrams of Depo-Medrol. The needle was then removed. The patient describes 5/10 pain prior to the procedure and 0/10 pain post procedure. No complications were encountered. 0.8 minutes of fluoroscopic time utilized.
