Wiki Neurosurgery-Microscope Spehnoidotomy

Neurosurgeon is doing a microscopic (not endoscopic) Sphenoidotomy w/ removal contents of sphenoid, then moving on to do transnasal pituitary tumor removal. There is no CPT for the spheniodotomy by this approach...Does anyone know if this would be included in the pituitary removal or should an unlisted CPT be used, or....?
 
I wouldnt bill seperately for the sinusus. He is going through the nose to remove the tumor this is pretty much part of his approach. I would bill 61548 and 20926 if he is taking an abdominal fat graft for repair.
