I am trying to find out what type of documentation the doctor needs to get 63030, 63056,51 paid. Also looking for 22612, 22630,51 and 63047 together. We are trying to get an appeal letter together but also trying to make sure we have to correct documentation. 63047 with 63030. HELP please. Thank you in advance for your help
