Hello,

So at the start of 2027 i see one aspect of the new guidelines will be charging e/m's for all routine ante visits and my question pertains to all the pregancies that are mid pregnancy at this time.....This will be an interruption of many pregnancy episodes. For example say normal pregnancy etc, my practice charges a 0502F no charge ante for routine visits up until delivery when we charge a global delivery charge. So will we just have to bite the bullet for all the visits that we won't get paid for that started in 2026 because i can't charge e/m's for them since these rules don't apply until 2027 but in 2027 i'm sure the delivery labor management codes they want us to use don't not have the work of antepartum care in them because we will be charging e/m's for individual visits prior to that point. So all visits prior to 2027 in a pregnancy episode just won't get any credit? I hope I'm explaining this good enough that you guys can understand what I'm getting at.... If we're able to back charge for them then what also happens to the ones that are past 90 timely filling?

If anyone has any information as to how this issue will play out or any good contact of who to ask that would be amazing!

in addition i'm going to assumed that i could then charge out all interrupted 2026-2027 antepartum care visits with either appropriate e/m's 1-3 visits or the 59425/59426 at the very end of the year all on 12/31 for all the antepartum care since the 2027 delivery codes will not include that so we don't miss loose out on being reimbursed for the care we gave for those patients in 2026
