Could someone please tell me what documentation the provider needs to bill for 99354 and 99355. I have a provider trying to bill for these and they only document that they spent 45 minutes with patient. They are trying to bill 99214 and 99354 is that enough time.

Also can they bill 99214 and 99495 together.
