To anyone's knowledge, have any payers ties the use of the new HFCA with the date of service. CMS has indicated that the use of the form is not DOS related, its for claim submissions in general as of 04/01/14.

My understanding is the new form is for all claim submissions as of April 1 without regard to visit date.

That totally makes sense. Just wanted to get some kind of verification before I get rid of the old forms so they are not accidentally used.

Even claims filed before April 1, 2014 but not paid, if you have to re-file or appeal, you must use the new form.
