Can you use New Patient History form to pull info into note, if it was signed and dated by provider on same date at appointment, for a New Patient. I say yes you can - but Medicare guidelines were have NGS as our carrier says it has to be notated. What exactly does that mean?? If you agree with me can you please send me some info to make my case with my supervisior.

The doctor just has to state/notate source, new patient history form, and describe ROS or what the information is. Patient for indicated family hx of....., personal history of....., etc. it is just quicker for doctor & everyone else to have the patient write this out prior to eval, and patient does not feel pressure to remember their diseases while talking to the doctor.
