If a new patient came in for wart and the documentation shows the doctor did wart destruction,
should I charge the destruction code only?
Or should I charge E/M (office visit) as well simply because he is a new patient?
In this case, is the E/M included in the procedure already?
Please advise. Thank you.
 
You can bill New Pt EM visit because the wart destruction was not planned ahead. Basically the doctor didn't know he was going to do it until he examined the patient. If he had seen this pt on a previous date and this was planned procedure than that's where you would only bill destruction code. You will most likely need to add a modifier to the EM code based on global period. Most likely the destruction has a 10d global period so adding 25mod is appropriate.
