A resident physician at a teaching facility sees a patient. After physician graduates from residency he/she then joins nearby clinic. Same patient is being seen again at new place of practice. Is patient new or established?
 
It depends on how long it has been since the patient has seen this physician, and if the physician had an existing medical record. Assuming the known patients visit in dr's newly established practice is within 3 years, I would be sure to code this established.
