My practice is a pediatric multi-speciality practice. We had a pediatric endocrinologist who left the practice about a year ago. She has just recently been replaced. Her patients with diabetes have been treated for the past year by either their PCP or an adult endocrinologist. These patients are now coming back to our practice to see the new doctor. She has access to the returning patients old medical record. My question is - are these returning patients consults, new patients or follow-up patients?
 
These would be return established patients as they saw a physician in the same practice within the last three years.
 
Established patients

I agree, as long they saw someone in the practice (even if that doctor has since left) within the past 3 years, they are established patients.
