Wiki New vs established pt - flu shots in past

We have a patient who recently came in for an E&M visit with one of our PAs, and she billed out a new patient visit. The patient has not been seen by a provider in over three years, but has been receiving his yearly flu shots at our practice, as recently as last fall. We're thinking that he should qualify as an established patient, but since he has not seen an actual provider we're not certain. Can anyone clear this up for us?

Thank you!
