T-19 and its MCO's do not reimburse for nipple tattooing when done outside of the global period, indicating that it is part of the nipple recon. All other commercial payers allow reimbursement for this, when its done outside of the global period of the nipple recon. Since, T-19 does do not allow pymnt, can we then charge the pt a self-pay amount?

Per CPT corner oct/nov 2013 Plastic Surgery News the tattooing is included in 19350 if the tattooing is done outside of the 90 day global then 19350 should be billed with 52 modifier. The reconstruction is then "completed" with tattooing which can then be reported with code 11921. If 19350 has been paid in full then the tattooing has already been paid and should not be billed to patient.
