If the date of service is not documented on the op report but the primary surgeon supplies a copy of his hcfa showing his billing. Can the assisting surgeon use that documented proof? I don't think it would stand in an audit.
 
Need DOS

The primary surgery should have addended the original op note to include a date of service. You MIGHT be able to provide other documentation (such as an OR log or anesthesia record that shows both primary and assustant surgeon present on that DOS).
