I have no clue how to code this dx

I am thinking that 724.9 "other specified back disorders" may work since there is not a specific code for what you have described.
 
The thecal sac is the protective enclosing membrane which surrounds the spinal cord and cauda equina. This sac is filed with cerebral spinal fluid and is often featured prominently on MRI imaging reports for back pain patients. Most people are not sure exactly where this sac actually exists in the anatomy or what it does. I don't know if I would consider this the spinal cord. 336.9 is more like cord compression or myelopathy.
