If a physician notes that the family history was reviewed or discussed but is non-contributory is credit given for the family history part of the encounter?
Nothing else is noted concerning the family history.

For most payers and MACs, no - this statement is not acceptable. I would check to confirm though. Most payers and MACs will give guidance on their page or through the Physician portals.
