I have a center where 2 non-medically directed CRNAs are doing cases (a lot of cataracts). The primary anesthetic is a block which is done with discontinuous time. One CRNA does all the blocks, the other does all the actual case time. We are capturing the block time and combining it with the case time (minus the break in time of course). Typically, the provider of record would be the one with the most time on the case (which in the following scenario would be CRNA#2). This could lead to overlapping times. How do others handle the start time to avoid this situation or does it not matter as long as the total time the CRNA is billing for is applicable (example, not over the 8 hours shift they work, etc)?

What should the initial time be when you have discontinuous time and you are reporting the total time?

For example:
Patient #1--CRNA #1 does the block from 12:00n to 12:10pm(10 minutes of MAC) and then CRNA #2 patient surgery is done from 12:45p to 13:00pm (15 minutes of MAC) total time 25minutes.
Patient #2?CRNA#1 does block from 12:15-12:25pm (10 minutes of MAC) and CRNA#2 does patient surgery from 13:05-13:20 (15 minutes of MAC) total time 25 minutes.

Any official guidance would be appreciated. I can only find info on medically directed scenarios.
