Wiki Non NCCN Treatment Deviations

Hello, I am looking for some insight into how other offices handle deviations from NCCN regimens? For example, would you allow treatment if the schedule was not as written in NCCN - tecentriq q 6 weeks instead of q 3 weeks? Would you allow opdivo + yervoy only if NCCN stated it needed to be given with carboplatin (Chemo)? Really needing an idea of if NCCN guidelines should be followed or if doctors can have their own discretion.
