Since Medicare (and advantage plans) will not accept "unspecified severity" codes for non-pressure ulcers, and there are no "unstageable" codes for non-pressure ulcers, can anyone give me some advice on how I can code this:

A diabetic ulcer is is noted 3 cm x 2 cm on the lateral aspect of the left 5th metatarso-phalangeal joint, completely covered with dry eschar, therefore, unstageable.*
 
You will need to chose the unspecified and appeal it if denied. This is a scenario where the documentation will support the unspecified option since there is no way for the provider to know the actual depth.
