pt comes for cough, fever, discolored phlegm and the HPI mentions pt also states she is not taking her Lexapro. PMH anxiety depression.
Final URI no more mention of the Lexapro do I still have to code a T and Z code even though the pt is not being treated for anxiety/depression? ?

There is nothing in what you've said here to support a T code or Z code - there is no adverse effect mentioned, and I would not assume that the patient is non-compliant simply because of a statement that the patient is not taking a particular medication as that could be due to any number of different reasons.
