I am trying to bill for a Novasure separately from the procedure since we are an Out of Network provider. Was thinking I should use HCPCS A4649, would this be correct? What kind of reimbursement should I expect?

Yes that would be correct. The product cost 1080.00 so hopefully you will get something close to that depending on the insurance and if they cover it.
