Wiki NP not credentialed bill incident to?

We have a NP starting and will not be credentialed. Can we bill "incident to" following all rules- not a new pt, not a new problem,in established plan of care, MD available? And does any other Insurance accept incident to or just Medicare? What about Medicare replacements?
